Which verb in bold should I use?
In the course of his work, he embezzled money from his company, and this led to him facing corruption charges. He was suspended from all duties and subsequently, my husband lost his job.
I was traumatised by this incident and I kept asking why I was so unfortunate, and how such an incident could happen/could have happened to our family? In everyone’s eyes, he is a good husband and a good father.

"could have happened" seems preferable. I think I would say "such a thing" rather than "such an incident". No question mark is needed on that sentence.
