What are you up to? asks about your current activity (=what are you doing?). It can also ask what are you capable of? [Can you run the entire mile?] It can also ask how far along are you? [You're trying for 500 push-ups; what are you up to now?]
What's up? is just as you describe as an informal greeting, with no literal meaning.
