My choices are in brackets.Please check it and correct if any mistake.
In negative sentences like (1) and (2) we do not use the word "sometimes".

You are right about negativity and 'sometimes' here, Hanuman, but they can appear together if differently ordered: 'I sometimes do not remember to brush my teeth'. All else seems fine.
