Stammer VS stutter

What is the difference between stammer and stutter? And also what do we call someone who stammers? A stammerer?

Speech therapists may consider the words differently, but in my mind they are the same phenomenon. However, to me a stuttering is the chronic disability, while stammering is what happens to many people in moments of stress or alarm-- that is just a personal observation, though. Yes, 'stammerer'.
