I would like to humbly request to use my annual leave pay for the days that I have been and will be absent from work. As you know that I have been away due to injury and will be back to work as soon as i feel better.

Words like humbly are not used by native speakers in normal business English. However I know that in some countries, elaborate politeness is very common, so you can include that if you wish to.

In Western cultures, many people get paid for days that they are sick and stay at home. These are referred to as sick-days or sick leave. A person might say, for example, I can take 10 sick days per year.
