SUBJECT: ACCIDENT REPORT ON Machine On the 9th July 2007 at about 1500 hrs, forklift truck went over the seaward side of Wharf No. 10 during operation and plunged into the sea below after knocking off the protective timber barrier at the edge of the wharf.
1.0 The Resurfacing Process The forklift truck was resurfaced the next day at 1530 hrs (please refer Attachment 1). 2.0 Technical Report on the Breakdown FL 222 As agreed in the meeting on 19th July 2007 chaired by Asst. Manager (Operation), the forklift was sent out to UMW Industries Sdn Bhd (UMW) workshop for inspection (please refer Attachment 2). The inspection was held on the 6th August 2007. Please proceed to Attachment 3 for the official report from UMW. 3.0 FL 222 Maintenance Record All repair works corresponding to previous complaints had been done. Up until the accident occurred, there were no complaints and outstanding works on the particular FL 222. 4.0 Recommendations A comprehensive quotation will be available as soon as possible as UMW is still pending for some pricings for some new items which have not been ordered before. However, it is suggested by UMW to consider a replacement for the damaged forklift.
