First I would like to thank you for the beautiful card you sent our family after Dad's passing. It meant the world to us, as you have been a big part of our lives for such a long time. Thank you for caring, beyond the role of the doctor -- you are a good man and an extraordinary physician.
Second, I would like to extend an apology for taking so long to send you this letter of thanks and appreciation. It has been almost three (3) months since we lost Dad. I have had to think long and hard for the words to tell you how much we love and appreciate all that you did for him. Every time I would put pen to paper, I could not find the words that would say thank you enough.
Dad was a walking miracle, we know how fortunate we were to have him as a long as we did. Dr. M, you gave us the information we needed to make decisions. You gave us time to mull things over, you empowered us and guided us and stood ready for next steps always...right up to the very end. For this we will forever be grateful.

This is a touching and moving letter which your doctor will welcome and appreciate. It is very well written. Any minor imperfections which we could point to would not detract from the sincerity of your feelings, and the doctor is not going to care about them.
