'The' may actually be preferable. It specifies surgery. It's not surgery in general, because some surgeries take less/more than one month before your're 'up and running'.

I think in BrE, if you found the sentence in a letter from a (rather informal) medical practioner, it would read either:
1. After surgery, you'll be up and running in a month.
2. After the operation, you'll be up and running in a month.
