Yes, but what good will it do to fill in a sample form? Why not send the actual form?

It sounds like this is some sort of practice session, and the real form will be sent later, to be filled out in the same way.

By the way, the usual expression is "Are my instructions clear?" I don't know why exactly, but we use the plural more often than not.
