This is a signed written authority allowing Mr. (full name) to accept a full bank statement from sort code&account number on my behalf for the dates --/--/---- : --/--/----.

If the bank has no record of your signature or if it is an old signature you may need to enclose a certified copy of your identification - usually a passport. Banks can be full of red tape in these matters.
