When did you 'realize' you could do nothing? Before or after changing your mind?

Only "realizing - is correct in this construction.

No, not unless you use "and":

I changed my mind and realized that ....

Otherwise it's considered a "run-on" sentence.

2) Then I changed my mind, having realized that he could do nothing to help.

Only slightly. having realized puts the realization slightly more prior to the change of mind.

For all practical purposes they are the same.
