Hi when I write a letter, When I call the receiver of the letter

I'm a bit confused about the difference between

Dear xxx

To xxx

So when do we use first one and the latter one?

Any significant differences?

So when do we use first one All the time, it's the standard way to start.

and the latter one? Never. Unless you want to write letters like:

To Mr. John Smith:

You have not paid your rent for the last year. If you do not pay by Thursday, I will change the locks on your apartment and throw your furniture into the street.
