Exclamation mark or comma?

"I hate you" she screamed.

I think the clause 'I hate you' should be followed by an exclamation mark as someone screams it. Am I right?

Yes. You need some punctuation there, and an exclamation mark is appropriate. "I hate you!" she screamed.
