Cousin

Okay so I wrote my older cousin a note because I have to move away closer to a hospital because I am possibly going to die soon so I wanted to write him my last note to him and I did and he cried I want to also give it to my friend but I don't want her to cry. So here's the note and I want someone to tell me if there is anything I should change...
Dear Anonymous,
Okay, so this is really hard for me to sit down and write you this note, but I have to tell you. I'm moving, and I'm most likely not coming back. I have been told that I am very unhealthy and sick. There is a possibility that I am going to die. I could get better, but it's going to take a little time and I still have to move. If I get better I will come back but there is a very small chance I will get better. Don't worry because I am not going to be completely gone. In fact I found a poem for you!
Do not sit at my grave and weep
I am not there I do not sleep.
I am the wind that continues to blow
I am the sparkle in the ice and snow.
I am the stars that shine at night.
The path I life is dark and I am your light. Just because you can not see me doesn't mean I am not here. I will ALWAYS be here for you. I'm sorry I have to leave you this way but I am going to try to get better for you I promise.
